Getting Started

Call and speak with me at 770-500-3585 or send me an email today about starting your Antiaging Medicine program.

STEP 1 – Contact Us

Once you have decided to pursue an Age Management Medicine program, simply complete the Patient Agreement (available on request) and FAX to 678.623-0041.

STEP 2 – Blood Test

Dr. Smith will arrange a blood draw for your initial laboratory evaluation either at my office or at your home which ever you prefer.

This very comprehensive lab panel will measure your hormone levels and biomarkers of aging.

You must be fasting for 8-12 hours before the test is performed - nothing to eat or drink except water.

STEP 3 – Medical History Form

Complete and return the Medical History Form while you are waiting for your lab results (this form will be emailed to you).

STEP 4 – Initial Consultation

Once I have your lab results and your completed Medical History Form we will schedule your initial consultation appointment.

Dr. Smith will spend about 120 minutes with you discussing the principles of bioidentical hormones, answering your questions, assessing your symptoms, taking a detailed medical history. Dr. Stewart will conduct a physical exam.

STEP 5 - Treatment

At the end of your initial consultation, you and your physician will determine the course of treatment. Should you opt for treatment your medications and supplements will be shipped to you the next day and every 28 days thereafter from our pharmacy.

STEP 6 – Follow Up

After 8 weeks of treatment, you will have your first follow-up consultation with Dr. Smith – this can be done at the office or by phone.

You will need to get a follow-up blood test 10 days before your consultation.

Dr. Smith will discuss the effects of the program and adjust your dosages accordingly.

You will repeat this process after 2-3 months and again every 6 months thereafter, depending on the situation.
